The patient himself will select “Consumer,” and any other representative will select “Surrogate.” Finally, input your CYMA Username and Default Password and click on the “Login” button. The Attendant’s Username will be their Unique ID, Consumer’s Username will be their Consumer Number, and the Surrogate’s Username will be using S instead of the first digit of the Consumer Number. For example: If the Consumer Number is 123456, the Surrogate’s Username is S23456. And the Default Password will be the last four digits of the user’s SSN + date of birth in MMDDYYYY format.
  • Change Default Password: Upon initial login, the CYMA system will prompt you to change the Default Password and set a new password with at least one uppercase letter, one lowercase number, and one special character between 12-24 characters.
